First-Time Visitor's Information
All the information you need to prepare for your visit and to make your visit as smooth as possible.

Bringing Required Information
Please bring the following with you on your first visit:
- Photo ID
- Insurance cards
- Referral card or slip
- List of current medications
- Medical and dental health history
- Completed medical/dental insurance forms
- Recent X-rays (within 6 months)
To ensure accurate diagnosis, it is very important that x-rays are recent (within six months). If you had x-rays taken with a previous dentist, you may request that they forward them to our office. If additional films seem necessary, they can be taken at our facility.
Important: Minors under the age of 18 must be accompanied by a parent or guardian during the visit.
During your visit

The first visit will start with a review of your medical and dental history and an initial examination. After a comprehensive examination, any dental findings will be directly discussed with you. Dental X-rays (pictures) are taken, if necessary, to ensure a thorough examination followed by oral hygiene instructions to emphasize proper oral health care. All aspects of a preventive dental health program including diet, hygiene, and fluoride will also be discussed.
For children 3 or older, a cleaning and polishing of their teeth will usually follow along with fluoride application. For optimal communication, we will explain each of our procedures in terms your child can understand. We want to make your child’s first appointment a positive experience in order to set the tone for future appointments. If any dental findings indicate preventative or restorative treatment, than subsequent appointments may be scheduled.

At our office, we make every effort to provide you with the finest care and the most convenient financial options. One way we do that is by helping you maximize your insurance benefits.
Important!
All insurance companies pay for orthodontic treatment over the course of the ENTIRE TREATMENT TIME GIVEN AT YOUR INITIAL CONSULTATION. (THEY DO NOT PAY IN ONE LUMP SUM.) Therefore, we highly recommend calling our office to confirm or verify that your plan has completed payments before you cancel your plan. If changing insurance plans, make sure that your plan covers “Treatment in Progress” – that way, your payments continue over to your new coverage. You may also call our office, and we will happily answer any questions or concerns you may have.
Insurance Discrepancies
Important: We cannot ethically guarantee any level of insurance coverage. Due to the complex nature of insurance companies and our lack of control, all we can provide you are estimates in good faith. Any quote you receive should be considered an estimate.
On the other hand, we can guarantee that we will work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. Furthermore, we do our best to safely overestimate our quotes; but please keep in mind that we can be wrong.
In the case of discrepancies between the estimate you received and your actual bill, you are fully responsible for the latter.
Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame.
